INFO FOR TRANSCRIPT OR REPORT CARD

One of the ministries that God has asked me to do is to work with CHA families to build their student's transcripts and/or report cards when they ask for them. Over the past few years, I have begun to streamline the process by tweaking the worksheets that I use, etc. This is the next step in that process. ***IMPORTANT REMINDER - CHA DOES NOT REQUIRE YOU TO SUBMIT GRADES, ONLY ATTENDANCE!! This is only a service to you.***

For the best results, fill out each field below as accurately as possible. You will see that I have repeated the fields for the Grade Level/Year and the Courses/Grades a total of four times. It is only mandatory for you to fill out the first fields for this form. I included four fields because most high school transcripts cover four years of work and many times I am receiving all four years at the same time. Please feel free to leave those fields blank if you are only sending one year's worth of grades. (Grade school parents, you can use those for each quarter if you wish.)

Grading Scales: There are differing opinions on grading scales in home education (go figure!). I will work with you to reflect yours; however, here are the two most used in the state of Florida. If you use one of these, choose it (below) and I will use it.

1 unit = 1 credit = 1 year

Weighted GPA:
AP or IB - add 1 point
Honors - add .5 point

Number of Volunteer Hours?

Those of you with High School Students, especially if you are trying for the Bright Futures scholarship or planning to go to college, putting the number of volunteer/service hours on the transcript is a good thing. Let me know if you have this number!
